Instead of being the customer requesting, wanting the upgrade (as fortunately is the case most of the time) we pushed for it. So anything that could be wrong from there, was of course our fault. "But in the old system it worked in this way and it was fine..." - everything went wrong from there. Doing the first step right, is crucial.
